Resident Rights

Every resident has rights protected under Oregon law. Residents receive a written copy of their rights at admission, and staff review them in a way that is easy to understand.

  • To be treated with dignity and respect
  • To privacy and confidentiality of personal and health information
  • To participate in developing their individualized service plan
  • To communicate freely, receive visitors and have access to a telephone
  • To manage personal belongings and, as appropriate, personal finances
  • To be free from abuse, neglect, retaliation and unnecessary restraint
  • To voice grievances without fear of retaliation

Grievance & Complaint Information

Residents, families and representatives may raise concerns at any time. We take every concern seriously and respond promptly and fairly, without retaliation.

  • Share concerns with any staff member or the Program Administrator
  • Written grievances receive a documented response
  • Residents may request help from a staff member or advocate to file a grievance
  • Concerns may also be directed to the Oregon Health Authority or Disability Rights Oregon

Privacy Practices

We protect the privacy of resident information in accordance with applicable federal and state privacy laws. A Notice of Privacy Practices is provided to every resident at admission and is available on request.

  • Health information is shared only as permitted by law or with authorization
  • Sensitive documents are exchanged through secure channels — never public web forms
  • Residents may request access to their records as permitted by law
