RTHs serve a small number of residents — PacificNest is designed for up to 5 adults — which allows for a more personal, less institutional environment.

PacificNest is designed for adults who benefit from a structured residential setting and ongoing behavioral-health support, including adults with serious or persistent mental-health needs who need assistance maintaining stability in the community. Every referral is reviewed individually; admission decisions are based on individual needs, program compatibility, available capacity and applicable regulatory requirements.

Yes, families are welcome to reach out. We will talk with you about the process and, where appropriate, help connect with the individual's case manager or treatment team, since coordinated referrals typically help ensure a smooth and appropriate placement.

The initial inquiry only requires basic referral contact information. If the referral moves forward, our team will request additional documentation — such as recent assessments and treatment information — through an appropriate secure process. Please do not send medical records through the website.

Yes. Medication administration and medication-management coordination are provided consistent with each resident's treatment plan, prescriber orders and applicable requirements.

PacificNest coordinates with licensed psychiatric providers for residents who require psychiatric evaluation or medication-management services. Specific arrangements depend on each resident's needs and existing providers.

Timing depends on available capacity, the completeness of referral information, assessment and transition planning with the referral source and treatment team. We aim to respond to inquiries promptly and will be clear about expected timelines. Submitting a referral does not guarantee admission.

PacificNest is located in Beaverton, Oregon, serving Washington County and the greater Portland metro area. To protect resident privacy, we do not publish the home's street address online. The location is shared directly with referral partners, residents, families and regulators as appropriate.
